Abstract | 「要旨」左前下行枝に有意狭窄を有する一枝病変症例89例について, 初回待機的PTCA前および3〜6ヵ月後の左室駆出率(LVEF), 局所短縮率(FS)を比較し, PTCAが心機能に及ぼす影響について検討した. PTCA後, 再狭窄(-)群ではLVEFが67.3%から70%(P<0.01), 前壁のFSが2.92%から3.39%(P<0.05)といずれも有意に増加したが, 再狭窄(+)群では改善は認められなかった. また, 心筋梗塞の既往を有する例ではPTCA後FSは1.86%から2.55%と有意に改善(P<0.05)したが, 既往のない例ではPTCA施行前からFSは良好であり, 有意な変化は認められなかった. 以上より, 心筋の収縮性が低下していてもviabilityが残っていれば, PTCAにより収縮性の改善が期待できると思われる. |

Abstract | To evaluate the effects of percutaneous transluminal coronary angioplasty (PTCA) on global and regional myocardial function, we studied 89 patients who underwent the first elective PTCA to the left anterior descending artery. Left ventricular function was measured from left ventriculography before and after PTCA. After PTCA, left ventricular ejection fraction (LVEF) and fractional shortening (FS) of the anterior wall significantly increased in the patients without restenosis. The improvement of FS after PTCA (from 1.86 % to 2.55 %;P<0.05) was not observed in the patients with history of anterior myocardial infarction (MI), although it was not observed in the patients with out anterior MI whose FS was normal before PTCA. In conclusion, these results indicate that reduced LV functiion may be improved by PTCA in MI patients, if myocardial viability remains in the area of MI and restenosis dose not develop.
