Abstract | 「要旨」一酸化窒素(NO)合成阻害薬を前処理すると, ニトログリセリンの血管弛緩反応は抑制されず, むしろその作用は過剰となる. そこで, 今回, NO合成阻害薬を慢性的に投与しても同様の過剰反応が生じるかどうか検討するために, NG-nitro-L-arginine methyl ester(L-NAME群)または生理食塩水(対照群)を10mg/kg/dayで14日間ウサギに投与し, ニトログリセリンの降圧作用を対照群と比較検討した. ニトログリセリン注入前のL-NAME群の収縮期血圧は168.3±2.9mmHgであり, 対照群の145.0±5.0mmHgに比べ, 有意に上昇していた. 拡張期血圧はそれぞれ91.7±7.6mmHg, 83.3±2.9mmHgであり, 有意差はなかったが, L-NAME群で高値を示した. ニトログリセリンの持続注入, 一回静注ともに, L-NAME群で対照群よりも血圧の低値を示した. 特に, 一回静注においては, 対照群で血圧低下後にリバウンド現象が見られたが, L-NAME群では見られなかった. また, 血圧低下時間も長かった. 以上のことから, NO合成阻害剤の投与下では, ニトログリセリンは過剰反応を起こし, 予期せぬ血圧低下を招く可能性がある. |

Abstract | To investigate the vasodilative action of nitroglycerin in white rabbits with chronic blockade of nitric oxide synthesis, the animals were divided into two groups. We infused NG-nitro-L-arginine methyl ester (L-NAME) intraperitoneally for 14 days at the rate of 10 mg・kg-1・day-1 using an osmotic pump, and in control group infused saline at the same rate, and then, we compared the changes of systolic blood pressure after administration of nitroglycerin with the control group. Systolic blood pressure before administration of nitroglycerin in the L-NAME group was significantly higher than that in the control group (168.3 ± 2.9 mmHg, 145.0 ± 5.0 mmHg, respectively). Diastolic blood pressure before administration of nitroglycerin in the L-NAME group was higher than that in the control group, but there was no significant difference. Systolic blood pressure after either a continuous infusion or a bolus injection of nitroglycerin in the L-NAME group was lower than that in the control group. Particularly, rebound phenomenon to vasodilation after a bolus injection of nitroglycerin was observed in the control group, but not in the L-NAME group. We conclude that nitroglycerin may cause the unexpected vasodilative action in the condition of chronic blockade of endogenous NO synthesis. (Circ Cont 17:554〜559, 1996) |
