Abstract | 「要旨」開心術後の脳障害は術後合併症として大きな関心が持たれている. 大量フェンタニール麻酔下に行われた中等度低体温体外循環使用開心術において復温速度, 復温時低血圧の術後脳障害に及ぼす影響をレトロスペクティブに検討した. 術後336例中42例(12.5%)に脳障害が発生した. 復温速度による術後脳障害発生頻度に有意差は認められなかった. 復温時における灌流圧50mmHg以下の時間×血圧である復温時tm50を復温時低血圧の指標とすると, 復温時tm50は脳障害発生群で高値を示し, 特に65歳以上の高齢者では有意差を認めた. 高齢者において復温時低血圧は術後脳障害発生の危険因子となる可能性が示唆された. |

Abstract | Cardiopulmonary bypass (CPB) is frequently complicated by postoperative neuropsycologic dysfunction. The aim of this study was to evaluate the effects of the rate of rewarming and hypotension during mild hypothermic CPB. Three hundred and thirty six were enrolled in this study, and the rate of rewarming and tm50 (MAP less than 50mmHg × time) in the rewarming period were recorded. The incidence of neuropsycologic dysfunction was 42/336 (12.5 %). The rate of rewarming and tm50 were unrelated to neuropsycologic dysfunction after cardiac surgery. However in elderly patients, tm50 was significantly higher in the neurologic dysfunction group. Our results suggest that hypotension during the rewarming period may contribute significantly to postoperative neuropsycologic dysfunction in the elderly. (Circ Cont 18:9〜14, 1997) |
