Abstract | 「要旨」ウサギを対象にニカルジピンおよびニトログリセリンによる低血圧麻酔が心筋虚血再灌流障害におよぼす影響を比較検討した. 全身麻酔下に開胸し, 左冠状動脈前下行枝の結紮による30分の虚血と180分の再灌流を行った. ウサギは無作為に3群に分類し, 薬物を投与しない群(n=6), ニカルジピンを投与した群(n=6), ニトログリセリンを投与した群(n=6)とした. 各薬物は虚血30分前から再灌流終了まで静脈内投与し, 収縮期血圧がベースラインの約80%になるように調節した. 術中は連続的に循環動態をモニタリングした. 実験終了後にはevans blueの染色により虚血域の大きさ(area at risk)を評価し, tetrazoliumの染色により心筋壊死域の大きさ(infarct size)を算出した. 各群の左室に対するarea at riskは各群で有意な差はなかった. Infarct size/area at riskはcontrol群55.1±3.5%に比較してニカルジピン群, ニトログリセリン群は各々36.8±1.7%, 57.1±2.1%でニカルジピンに心筋壊死縮小効果が認められた. 以上より, 少なくとも両薬物が心筋壊死域を増大させることはなく, ニカルジピンは, むしろ心筋に保護的に作用する可能性が示唆された. しかし, 循環動態的にはニトログリセリンは心拍数を増加せず, 拡張期血圧の低下も軽度であった. |

Abstract | This study was designed to elucidate comparative effects of induced hypotension with nitroglycerin and nicardipine on the intensity of myocardial necrosis (infarct size) following ischemia and reperfusion. Rabbits (n=18) received regional ischemia by 30 min of the left anterior descending artery (LAD) occlusion followed by 3 hrs of reperfusion. under ketamine/xylazine anesthesia. The animals were randomly assigned to a control group, and nicardipine or nitroglycerin treatment groups, in which a continuous infusion of nicardipine (0.12mg・kg-1・h-1) or nitroglycerin (0.06mg・kg-1・h-1) was initiated 30 min prior to ischemia. The systolic arterial blood preeesuure was adjusted to about 80% of the baseline values and maintained throughout ischemia and reperfusion. The area at risk was delineated by Evans blue, and infarct size was detemined by tetrazolium staining at the end of the experiment by religation of LAD. The area at risk showed no significant differences among three groups. Infarct size/area at risk in the nicardipine rabbits (36.8 ± 1.7%) was significantly smaller than those of control (55.1 ± 3.5%) and nitroglycerin (57.2 ± 2.1%) rabbits. |
