Abstract | 本研究は, in vivoウサギ心筋虚血再灌流モデルを用い, ischemic preconditioningおよびイソフルランによるpreconditioningが心筋虚血再灌流障害に及ぼす影響を循環動態, 虚血域(以下area at risk)および心筋壊死域(以下infarct size)を指標とし検討した. ウサギ(n=23)をケタミン/ザイラジンによる全身麻酔下に開胸し, 左冠状動脈前下行枝を結紮し30分間の虚血を行った後, 180分間の再灌流を行った. 実験中は血圧, 心拍数, 心電図を持続的にモニタリングした. 実験群はコントロール群(C群:n=8), ischemic preconditioning群(IP群:n=8), イソフルランによるpreconditioning群(I群:n=7)とした. Infarct sizeは1% 2,3,5-Triphenyl tetrazolium chloride(以下, TTC溶液)による染色で評価した. infarct sizeはC群が52.2±1.6%に対し, IP群36.3±2.6%, I群39.8±1.9%(mean±SEM)であり, IP群とI群において心筋壊死域は有意に減少した. 以上よりウサギ心筋虚血再灌流モデルにおいて心筋虚血前に一時的に投与したイソフルランはischemic preconditioningと同様に心筋に対して保護的に作用し, そのメモリー現象は少なくとも30分は持続する可能性が示唆された. |

Abstract | We evaluated the effects of ischemic preconditining and Isoflurane induced preconditioning on myocardial infarction within a quantified area at risk in rabbit hearts subjected to a regional ischemia/reperfusion injury. In-vivo rabbit hearts(n=23) were received regional ischemia by 30min of the left anterior descending artery occlusion and followed by 180min of reperfusion under kctamic/xylazinc anesthesia. Rabbits were randomly devided into three groups : Group-C (Control, n=8) were given no pretreatment, Group IP(Ischemic preconditioning, n=8) were pretreated with 5min of coronary occlusion followed by 10min of reperfusion, Group-IP (Isoflurane induced preconditioning, n=7) were pretreated with 15min of isoflurane(1.5% end-tidal) inhalation and washout. The infarct size determined by tetrazolium staining was: 52.2±1.6% in Group-C, 36.3±2.6% in Group IP, 39.8±1.9% in Group-I. The infarct size was smaller in the Group IP and Group I than in the Group-C (p<0.05, respectively). The administration of 1.5% isoflurane before LAD occlusion resulted in a reduction in myocardial infarct size similar to that observed with ischemic preconditioning in an in vivo rabbit model, suggesting the presence of preconditioning effect in isoflurane. |
