| Abstract | 「要旨」脳梗塞は以前と比較すると減少傾向にあるが, 依然として, 統計的に死因の上位を占めている. 著者らは, 陳旧性心筋梗塞の既往のある左後大脳動脈と左上小脳動脈の両血管系に同時発症した脳梗塞の1例を経験した. 画像形態, 発症様式から, 脳塞栓と考えたが, 塞栓源は同定し得なかった. 「はじめに」多発性脳梗塞の大半はラクナ梗塞であり, 初回に梗塞を起こしたのち, 再発することが多い. 著者らは椎骨動脈系に同時発症した稀有な脳梗塞症例を経験した. 症例：58歳の男性. 右手利きであった. 主訴：回転性眩暈 既往歴：陳旧性心筋梗塞(22年前に発症：下壁, 9年前に発症：心内膜下). 高血圧, 高脂血症, 糖尿病の既往は認めず, 22年前の下壁梗塞後から1日に81mgのアスピリンを服用していた. 喫煙歴：20本/日×20年 家族歴：脳血管障害を含め特記事項なし 現病歴：本年3月昼食直後, 午後0時30分頃, 突然の回転性眩暈, 嘔気, 頭痛, 軽度の構音障害, 右半身のしびれがほぼ同時に出現し, 座り込んでいるところを同僚に発見され当科に救急搬送された. 入院時身体所見：身長164cm, 体重60kg, 血圧92/60mmHg, 心拍数72回/分整, 呼吸数14回/分, 心音に異常なく心雑音は聴取せず, 呼吸音は清明, 末梢動脈の触知は良好で下肢静脈瘤は認めず, 頚部血管雑音なし |

| Abstract | We encountered a patient with cerebral infarction developing simultaneously in both the left posterior cerebral artery and the left superior cerebellar artery. The patient had a history of myocardial infarction. Based on diagnostic images and the type of disease onset, the case seemed to be cerebral embolism. However, no source of embolism could be identified. |
