| Abstract | 「要旨」局所麻酔薬レボブピバカインは, ブピバカインのS(-)体として開発された薬物である. 臨床では, 局所麻酔薬の血管内誤投与による心毒性が問題となる. そこで, ラット摘出心臓を用いた灌流実験を行い, レボブピバカインの心刺激伝導系を介した心機能抑制に対するペーシング処置の効果, および, カテコラミン(エピネフリン, ノルエピネフリン, イソプロテレノール)による心機能抑制に対する改善効果を検討した. ペーシング処置においては, 5μg/mlのレボブピバカイン濃度による心機能の抑制が緩和され, カテコラミン製剤投与においては, エピネフリン0.8μg/kg/min投与で心機能抑制が緩和された. |

| Abstract | Levobupivacaine is a long acting, amide-type local anaesthetic that is the S(-)-isomer of the racemate bupivacaine. It has high affinity for the myocardial Na+ channel that can be cardiotoxic. So, an intravenous injection of levobupivacaine sometimes becomes a serious problem as the inhibition of cardiac function. Then, we investigated the effects of cardiac pacing and catecholamines(epinephrine, norepinephrine, isoproterenol)to treat the inhibition of cardiac function in isolated rat hearts. Our results showed that cardiac pacing and 0.8μg/kg/min epinephrine recovered the inhibition of cardiac function due to 5μg/ml levobupivacaine. |
