Abstract | 『緒言』 感染性胸部大動脈瘤は比較的稀であり診断・治療に苦慮する. 今回, 感染性弓部大動脈瘤に対して弓部置換術と大網充填術を施行し良好な結果を得たので報告する. 『症例』 症例:56歳, 女性. 主訴:背部痛, 発熱. 既往歴:糖尿病, 気管支喘息. 家族歴:特記すべきことはない. 現病歴:平成×年1月7日に背部痛と発熱を主訴として近医を受診した. 気管支喘息の既往から点滴治療施行して一時改善した. しかし, 症状が再燃したため1月20日に受診し, CTで大動脈弓部に嚢状瘤を認めた. 2月3日に当院を紹介受診し, CTで瘤の拡大傾向が認められたため手術目的に2月5日に入院となった. 入院時現症・検査所見:身長149cm, 体重51kg, 血圧119/68mmHg, 脈拍98/分, 心雑音なし. 血液検査所見では白血球10,210/μl, CRP 10.02mg/dlと炎症反応を示し, 空腹時血糖値179mg/dl, HbA1C 11.1%と血糖のコントロールは不良であった. |

Abstract | A 56-year-old female was admitted to a hospital due to fever and back pain. She was diagnosed of asthmatic bronchitis and received antibiotic therapy. But two weeks later she complained of back pain again. Her white blood cells and C-reactive protein(CRP) were elevated and computed tomography(CT) revealed aortic arch aneurysm. Two weeks later, she was referred to our hospital. CT revealed rapid enlargement of the aneurysm. She was diagnosed of mycotic descending aortic aneurysm and underwent resection of the aneurysm and in situ prosthetic graft replacement and omentopexy around the graft. She received antibiotic therapy until the day 52 postoperatively. Their postoperative courses were good and no recurrence of infection was seen. |
