| Abstract | 「要旨」1977年8月から1984年12月までに経験した3003例の開心術症例中低心拍出量症候群(LOS:心係数<2.2l/min・m2)に陥った669例を対象に, LOSによって惹起される他の臓器障害について検討した. 合併臓器障害で最も頻度の高いのは呼吸障害でLOS症例の49.9%で次いで腎障害が29.9%, 以下肝障害(18.4%), DIC(16.4%), 中枢神経障害(15.5%), 消化管出血(11.1%)の順であった. 呼吸器障害合併の死亡率は36.8%, その他の臓器障害合併例の死亡率はいづれも50%以上できわめて高率であった. さらに, LOSからのMOF進展様式およびLOSに最初にみられる臓器障害である呼吸不全および急性腎不全について概説した. |

| Abstract | During the periods from Aug. 1977 to Dec. 1984, 3003 patients received open heart surgery at National Cardiovascular Center and 669 of them developed low cardiac output syndrome (LOS) postoperatively. Various types of organ failure were observed in these patients with LOS such as respiratory failure (RF : 49.9%), acute renal failure (ARF : 29.9%), hepatic failure (HF : 18.4%), disseminated intravascular coagulation (DIC : 16.4%), failure of central nervous system (CNSF: 15.5%) and gastero-intestinal bleeding (GIB : 11.15). The mortality of LOS patients associated with an organ failure was very high, where 36.8% of patients with RF, 54.5% with ARF, 52.8% with HF, 59.1% with DIC, 62.5% with CNSF and 66.2% with GIB were accounted. |
