Abstract | 「要旨」ターニケットによる駆血解除時の患者の状態を呼気終末炭酸ガス濃度(ETco2)から推測するために, 片側膝関節置換術予定患者10例で, ETco2を連続的にモニターした. 麻酔は腰部硬膜外麻酔と吸入麻酔を併用し, 一定の換気条件を駆血解除後測定終了時まで維持した. その結果, 収縮期血圧は駆血解除2分後に最低値となり, pHは駆血解除1分後から低下して3分後には平均7.36となり乳酸も増加した. この間, ETco2は駆血解除から平均30秒経過してから上昇し, 駆血解除直前の34.29±1.39mmHgから解除後130秒で最高値38.85±1.23mmHgまで変化した. 以上より, 術中と同じ条件で調節呼吸を継続すれば, ETco2の変化が, ターニケットによる駆血解除時の循環系や代謝系の変動に対する指標となることが示唆された. |

Abstract | In this study we evaluated the cardiovascular and metabolic responses to toruniquet release by monitoring the change of end tidal CO2 continuously in 10 patients undergoing lateral knee replacement surgery. Anesthesia was maintained with both of inhalation anethetics and epidural anesthesia. After tourniquet deflation, controlled ventilation was maintained without change during measurement. There was a statistically significant decrease in systolic blood pressure (after 2 min.) and in pH value (after 3 min.). There was also a statistically significant increase, in concentration of lactec acid. During these changes, the end tidal CO2 increased significantly from 34.29±1.39mmHg to 38.85±1.23 mmHg (130 sec. after tourniquet deflation). This study shows that, under the controlled ventilation maintained without change, cardiovascular and metabolic responces to tourniquet release can be evaluated by the value of end tidal CO2. |
