Abstract | 「要旨」無機能性副腎腫瘍に対する腹腔鏡下副腎摘出術の麻酔管理を経験した. その際従来のモニター以外に動脈内血液ガス持続モニタリングシステム(PB3300)を使用し, 気腹時の急激な肺酸素化能の変化を連続的に記録し本システムの有用性を検討した. 麻酔法は酸素, 亜酸化窒素+イソフルランとした. 麻酔導入後, 左側臥位に体位変換し炭酸ガスを用い気腹を開始した. 気腹開始直後より経皮的酸素分圧(SpO2)が100%から93%と急激な低下を来たし, その際, 本モニタ上の酸素分圧(PaO2)は85mmHgを示した. ただちに吸入酸素濃度を上昇させたところSpO2, PaO2共に速やかに上昇した. また, 術中動脈血採血を14回行い, 採血して得られた血液ガス実測値と同時点でのPB3300から得た血液ガス値とを比較検討した結果, pH, PaO2, PaCO2ともに, 両測定法間で有意な相関を認めた. PB3300は麻酔管理上リアルタイムな呼吸モニタとして極めて有用であった. |

Abstract | We experienced anesthetic management of laparoscopic adrenalectomy for nonfunctioning adrenal tumor in a 68 year old female. Rapid changes of pulmonary oxygenation were continuously recorded during pneumoperitoneum, using an arterial blood gas continuous monitoring system. Anesthesia was maintained with nitrous oxide 60% and isoflurane 0.5-2.5% in oxygen. After induction of anesthesia, the body position was changed to the left lateral, and pneumoperitoneum and Pao2 of the monitoring system was 93mmHg. Immediately after increasing in Fio2, Spo2 and Pao2 returned normal. Intraoperatively, arterial blood samples were collected 14times, and the actually measured levels of blood gas (A) were compared with the blood gas levels simultaneously obtained from PB3000 (B). The results showed a significant positive correlation in pH, Pao2 and Paco2 (r2>0.91) between A and B. It is concluded that PB3000 in an useful tool as a realtime respiratory monitoring during anesthetic management for pneumoperitoneum. |
