Abstract | 「要旨」頚動脈血栓内膜摘除術(CEA)中に施行される一側内頚動脈遮断に際し, 同側の内頚静脈酸素飽和度(Sjvo2)を測定し, 同時に測定された体性感覚誘発電位(SEP)及び内頚動脈断気圧(SP)と比較検討した. 内頚動脈遮断時のSEP振幅の低下率とSjvo2値の低下率には明らかな相関が認められた. また, SPが30mmHg以下の症例では遮断時にSjvo2値の明らかな低下が認められた. 内頚静脈の走行異常が認められた症例ではSjvo2値は予想される変化を示さなかった. 以上より, Sjvo2はCEA中の脳血流のモニターとして有用と思われるが, 内頚静脈の形態異常が認められる症例ではその評価に注意が必要と考えられた. |

Abstract | By monitoring the jugular venous oxygen saturation (Sjvo2), we investigated the adequacy of collateral cerebral circulation during cross-clamping of the internal carotid artery in patients who underwent carotid endarterectomy (CEA). In 11 patients, Sjvo2 on the operated side, SEP and stump pressure of the internal carotid artery were measured and the relation among them were examined. Changes in SEP and Sjvo2 during clamping of the internal carotid artery had a good correlation. Cases with a stump pressure of higher than 30 mmHg, Sjvo2 maintained satisfactory throughout the clamping period. On the other hand, cases with a stump pressure lower than 30 mmHg were found to show dcreased Sjvo2 after clamping the internal carotid artery. In the one case with the anomaly of cerebral veins, despite decreasing rSO2 after cross-clamping of the internal carotid artery, Sjvo2 maintained high level during the clamping period. It is concluded that Sjvo2 monitoring is a useful indicator of cerebral oxygenation during crossclamping of the internal carotid artery in CEA, however, in the cases with the anomaly of cerebral veins, proper attension will be important to assess Sjvo2 values.
