Abstract | 「要旨」人工心肺を使用しない心拍動下冠動脈バイパス術において, 吻合操作を容易にするために心拍数を低下させる方法としてジルチアゼム(DTZ)の効果を検討した. 対象症例は8例で, 平均年齢は70歳であった. 胸骨正中切開後よりDTZを0.5μg/kg/minで開始し, 心拍数に応じて投与速度を決定した. その結果投与前は心拍数84±20bpmであったが, DTZ最大投与3.3±1.3μg/kg/minにて冠動脈遮断前は63±12bpmに有意に低下(P<0.05)させることが可能であった. また心拍数低下にもかかわらず遮断前, 遮断中にも血圧, 心係数, 混合静脈血酸素飽和度は維持され, 我々のDTZ投与法は心拍数を低下させる方法としては安全かつ有用と考えられた. しかし直ちに心拍数を低下させる必要がある場合, DTZには速効性の点で問題があり, 他剤の併用投与が必要と考察された. |

Abstract | We investigated heart rate (HR) change induced by diltiazem (DTZ) to ease anastomosis manipulations in eight patients who underwent coronary artery bypass grafting (CABG) without cardiopulmonary bypass (CPB). The average age was 70 years. Continuous infusion of DTZ was started at the rate of 0.5μg/kg/min after the sternotomy, the infusion rate was gradually raised up according to the HR change. In a result, at the maximum infusion rate of 3.3±1.3μg/kg/min, the mean HR was significantly (P<0.05) decreased from 84 ± 20 bpm before the infusion to 63 ±12 bpm before the clamp without hemodynamic deterioration. We concluded that continuous infusion of DTZ was safe and effective for decrease in HR. However, in case that immediate decrease in HR would be needed, DTZ seemed to have the disadvantage of rapid effect, therefore, other drugs should be needed in addition to DTZ.
