Abstract | 「要旨」アムリノンを開心術症例に使用し, 血小板数に対する影響及び術後出血量との相関を検討した. 対象は待期的冠状動脈バイパス手術22症例で, 投与群(A群)12例, 対照群(C群)10例であった. 患者背景, 術中データーに有意差はなかった. アムリノンは, 人工心肺離脱時に1mg/kgボーラス投与し, 続いて10μg/kg/minを持続投与した. 平均投与量は7.43±0.95μg/kg/minで, 投与期間は26±5時間であった. 血小板数は手術終了直後でA群9.35±1.72×104/μlに対しC群11.71±2.10×104/μlで有意にA群が少なかったが(P<0.05), 以後は同様に推移した. 総輸血量, 術後ヘマトクリット値及びドレーン排液量は両群間に有意差はなかった. アムリノン投与による術直後の血小板減少は軽度で, 出血量との関連は見られなかった. |

Abstract | The thrombocytopenia and postoperative bleeding resulting from administration of amrinone, a new inotropic and vasodilator agent, was evaluated. Patients were randomized to receive no amrinone(n=10), or amrinone(n=12)at a loading dose of 1mg/kg in the CPB circuit followed by 10μg/kg/min. Preoperative , pletelet count and hematocrit levels were similar in two groups, and the standard surgical procedures were performed. The duration of administration of amrinone was 26±5hr and total amount of amrinone was 14.2±3mg/kg. The platelet count was significantly reduced in amrinone group only at the end of surgery (9.35±1.72 versus 11.71±2.10×104/μl ; mean±SD, P<0.05). But there were no significant differences in chest tube drainage or hematocrit levels postoperatively between the groups. In conclusion, thrombocytopenia due to amrinone was slightly and did not attribute to the amount of postoperative bleeding. |
